Welcome To My Practice

What to expect at your first visit

A new patient appointment is 90 minutes. Return visits are 45 minutes (for most insurance coverage) to 60 minutes. Longer 90 minute sessions are also available. My office is on the second floor, stairs only. There is a parking lot on the south side of the building. The fourth slot in is labeled WENDY, and that’s for my clients to park in. There is also free on-street parking in the neighborhood. When you arrive, I will meet you at the front door of the building to let you in. I’ll be downstairs around the start of your appointment time. If you arrive early or are running late, please text or call me to let me know. I do not check emails in the treatment room or on my cell phone.

Patient Forms

Before your first office visit, please complete both the Client Information and Patient Intake forms linked below. Please bring the completed paperwork with you or email  forms to [email protected] at least 48 hours prior to your session.

Please allow 20 – 30 minutes to complete paperwork.

Mask Policy

I work with people who are immunocompromised, have comorbidities and Long COVID. With the highly infectious nature of circulating respiratory viruses, please do NOT come in if you or any members of your household are actively:

  • Sick – coughing, sneezing, sore throat, cold and flu symptoms, COVID 19 symptoms (fatigues, loss of taste or smell, skin rash, cough, congestion, body aches, fever, pink eye, nausea, diarrhea, abdominal pain, etc.) Best to stay home and rest!
  • If you have been in contact with anyone diagnosed with COVID 19 in the past 14 days, please reschedule your appointment.
  • Are testing positive for COVID 19 even without symptoms.

I appreciate your attention to these details. Masks are required in my office. Please wear a well-fitting KN-95 or KN-94 or surgical mask to the appointment. I’ll be wearing a mask and running an air purifier in the treatment room. I have hand sanitizer available for you to use. I spray and wipe down the treatment room in between each patient. It’s important for me to keep myself, my family and you all healthy and safe in my office! Your safety and comfort is foremost in my mind.

Medical Insurance and Billing Information

I am in network with the following insurance companies (this list may not be complete, and may change):

Aetna
First Choice
Kaiser Permanente Washington state
Regence BlueShield
Regence Group Administrators
United Healthcare
Personal Injury Protection with Auto Insurance

**Please call to verify your acupuncture coverage with your insurance carrier prior to treatment. While Medicare states it covers acupuncture, my billing experience has been that they often deny payment, and the client is financially responsible for the session.

Cash Rates

For clients not using insurance, please contact me for current rates.

Payments and co-pays are expected at the time of your visit. I accept checks, cash, Zelle ([email protected]) or Venmo (@Wendy-Mintiero)
